UNDERSTANDING YOUR JOURNEY
Once you have chosen Beyond Collaborative Care as “My Provider” the following happens:
1. Choose Your Provider:
Your referral is received, and a service agreement is sent out to your nominated people to review and sign.
2. Referral:
Once we receive your signed service agreement and plan manager approval, your NDIS therapist will contact you to arrange services.
3. Service Beginning:
An NDIS contact will offer an implementation meeting that can help you choose providers, understand and manage your plan, set up services, and work towards your goals.
4. Using your Plan:
The participant journey outlines the different steps that people will experience once they have their NDIS plan.
5. Getting Started:
All NDIS participants (new NDIS plans or existing) will meet with the NDIA planner who approves their plan.
6. Plan Meeting
NDIS plans will remain in place until a participant’s situation or support needs change. Some plans will be up to 3 years, however children under 9 plans are normally for 1 year.
7. Length of a Plan
